The RCFE application is a package, not a single form. It asks the state to confirm four things: who is operating the facility, who is administering it, whether the building meets requirements, and how the home will run day to day.

What goes in the package

Expect to provide operator and administrator information, proof of administrator certification, a program plan describing how you will care for residents, facility documents covering the physical plant and fire clearance, and financial information showing you can run the home. The exact forms are set by the Department of Social Services and are updated from time to time, so working from the current versions matters.

Where applications stall

Most corrections trace back to three things: incomplete or outdated forms, a facility that is not yet ready for the pre-licensing inspection, and missing administrator certification. Preparing these in the right order, rather than all at once, is what keeps an application moving. Assembling that package in the right order is a large part of the work we take on for clients.
